The balanced harmony of the facial skeletons seems to be the most important prime factor in the beauty of the face. In fact the previous esthetic surgical techniques have been pursued primarily the correction of the soft tissue of face. Recently there have been many trials of changing facial skeletons with the intention of achieving facial beauty.
The basic cause of the skeletal malformation in "horse face", in other terms, "long face syndrome", "vertical maxillary excess", "maxillary alveolar hyperplasia" is an excess in the vertical dimension of the maxilla. In 1974 Willmar first presented a follow up study of 3 operated patients with long face deformity. He removed excess maxillary segment by way of LeFort I osteotomy. We have presented a result of treatment of long face syndrome patient with gummy smile, in which crse, we used LeFort I osteotomy and excised 7mm band of maxillary segment and simultaneous genioplasty. Six months later augmentation rhinoplasty was added for the correction of changes in nasal contour after maxillary ostectemy.
